Children’s System of Care
Children’s Partnership
Project Manager - Sonia Hartman
The Children’s Partnership is a collaborative of the public child
serving agencies in Travis County including Juvenile Probation and the Community
Resource Coordinating Group (CRCG), and Travis County MHMR, the Texas Department
of Family and Protective Services (DPRS), and Travis County Health and Human Services & Veterans Services.
The Children’s Partnership leverages federal, state and local funds to build a system of care for severely emotionally disturbed children that is :
- child centered and family focused with the needs of the child and family dictating the types and mix of services provided,
- community based, with the focus of services as well as management and decision-making at the community level, and
- cultural competent and responsive.
This system of care involves a strength-based, holistic wraparound approach to providing community-based services and preventing the need for residential placement. HHS&VS provides a community home for the Children’s Partnership independent of the mental health authority that provides the fiscal agent, managed services organization and care coordination functions for the initiative. The OCS is responsible for the project direction, grants administration, quality assurance, and technical assistance functions for this project.
Youth & Family Assessment Center
Project Manager – Laura Peveto
The Youth and Family Assessment Center (YFAC) is a collaborative initiative to expand the community’s system of care efforts and provide this strength-based, holistic wraparound approach to children and youth at-risk of serious behavior problems and school drop-out.
The initial phase of YFAC utilizes a common assessment and managed services process to provide a full continuum of services for children, youth and families in and around Johnston High School feeder pattern in central east Austin and the middle schools in and around Reagan High School feeder pattern in Northeast Austin.
The result is both stronger families and demonstrated improvement in a youth’s functioning in school, the community and at home. Partners for implementation of the service delivery approach include:
- Travis County HHS&VS
City of Austin- The Austin Project,
- Communities In Schools
- Austin/Travis County Mental Health Mental Retardation
- Travis County Truancy Court
- Travis County JPO
- Region XIII Educational Services Center, and
- AISD.
Triad
Program Case Manager – Gloria Petersen
Triad is a collaboration of Travis County Juvenile Probation Department, Austin/Travis County Juvenile Court and Children’s Protective Services. These agencies coordinate wraparound services to children who have no other source of payment for residential treatment and would otherwise fall through the cracks in the children’s services delivery system.The program purchases residential treatment and related services for children and provides case management support to these children and their families.
