Project Description:

Home-based case management services to households residing in the unincorporated areas of the county focusing on precincts 1 and 4. Services include case management, information and referral, non-clinical counseling, crisis intervention and outreach.

Approximately 120 persons per year

Objective: Improve the availability of services for low/moderate income persons

PY 07:   $ 27,010.28 invested, serving 71 people over a 6 month time frame (12 month goal 100 people).

PY 08:   $ 64,293.50 invested, spent serving 137 people over a 12 month time frame (12 month goal 100 people).
